Advanced Nurse Practitioner/ Allied Health Professional (Nursing and Care Homes)

  • Salary: £39,878 to £45,753 per annum pro rata, depending on experience
  • Hours: Up to 30 hours per week, to be discussed at interview

  • Contract: Permanent

An exciting opportunity has arisen at our established and busy practice for an experienced Advanced Nurse Practitioner/Allied Health Professional to join our team at the Phoenix Health Group.

The Phoenix Health Group has been offering an enhanced care home service for nearly two years. The role has moved away from traditional reactive models of care delivery; towards proactive care that centres on the needs of the individual resident and their families. The overall aim is to improve the quality of care they receive, ensuring optimum and consistent access to, and provision of NHS services.

The improved collaborative working between Healthcare Professionals, Care Home Partners and the use of urgent community services in the area; has resulted in more positive outcomes for the residents in terms of their (and their family) personal preferences and avoidance of inappropriate emergency admissions to acute and community hospitals.

The post holder will be an experienced Nurse or Allied Health Professional, who, acting within their professional boundaries, will provide personalised care for each resident of their allocated care home, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their continued care. They will demonstrate safe, clinical decision-making and expert care for residents in care homes. They will work collaboratively with other community health care providers, hospitals and the care home partners to meet the needs of residents, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Administrator/Scanner

  • Salary: £8.72 per hour
  • Hours: Part-time, 16 hours a week. Hours to be worked over a choice of Monday, Thursday and Friday (Thursday must be a minimum of 7 hours)

The purpose of the role is to:

Support the Practice to move efficiently towards becoming a paper-light environment by way of scanning, electronically labelling and distributing correspondence to relevant clinical staff members.

You will need:

  • Excellent communication skills, written and oral with the ability to communicate confidently
  • Competent user of Microsoft Office software
  • An understanding, acceptance and adherence to the need for strict confidentiality
  • Ability to use own judgement, work without direct supervision and determine own workload priorities

Lead Urgent Care Practitioner

  • Salary: Up to £45,753 per annum, depending on experience
  • Hours: 37.5 hours per week, Part time/Job share would be considered

We are seeking a dynamic and confident Urgent Care Practitioner for this exciting new role at Phoenix Health Group.

We are developing a new urgent care team across our forward thinking, large multi-site practice, and are looking for someone to join us and help put our vision in to practice. The role would suit an ANP, Paramedic or Physicians Associate with the right leadership skills. We will then complement their skills as we build a balanced team to support our doctors to provide outstanding care to our patient population.

You will be acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care with the support of their named GP.  You will demonstrate safe, clinical decision-making and expert care for patients within the general practice. You will work with the multi-disciplinary general practice team to support the delivery of policy and procedures and meet the needs of patients.

  • Salary: £21,895 to £22,703 per annum (£11.38-£11.80 per hour) depending on experience

As a Care Coordinator you will work closely with our GPs, supporting them to provide personalised care to their patient population.

You will help people to manage their needs through answering queries, supporting them to make and manage necessary appointments with community and secondary care. You will ensure appropriate follow up and support with the referral process; and that they have good quality written or verbal information to help them make informed choices about their care.

You will need:

  • Experience in a healthcare setting
  • Active and empathic listening with excellent communication and questioning skills
  • Ability to build trust and rapport
  • Ability to use your own judgement, resourcefulness and common sense
  • Ability to work without direct supervision and determine own workload priorities
